Clinical trials are undergoing a transformative evolution driven by technological innovations aimed at improving patient outcomes, operational efficiency, and data integrity. With rising complexity in diseases and increasing demand for precision medicine, traditional clinical trial models are no longer sufficient to address the multifaceted challenges of modern research. The integration of digital technologies such as artificial intelligence (AI), wearable devices, blockchain, and decentralized platforms has enabled a paradigm shift toward more adaptive, inclusive, and patient-centered trials. This study explores the implementation of an AI-enhanced clinical trial framework combining machine learning algorithms, real-time data acquisition via wearables, adaptive trial designs, and blockchain-secured data management. A hybrid methodology was adopted, wherein clinical trial simulations incorporated Q-learning models for treatment optimization, mobile health tools for adherence monitoring, and cloud infrastructure for multisite data aggregation and analysis. Results indicate a significant improvement in clinical trial performance across multiple dimensions. Wearable-monitored trials demonstrated higher adherence and retention rates, while AI-supported recruitment and protocol adjustments led to shorter trial durations and enhanced safety monitoring. Ethical compliance improved through the adoption of e-consent platforms and blockchain-based data logging, minimizing privacy breaches and protocol violations. Moreover, personalized treatment regimens supported by AI showed increased response rates in genetically stratified cohorts. In conclusion, the convergence of AI, digital health technologies, and regulatory innovation is reshaping the clinical trial ecosystem. These advancements promote not only methodological efficiency but also ethical accountability and patient empowerment. As this technological synergy continues to evolve, it offers unprecedented potential to accelerate the development of safe, effective, and equitable therapeutic interventions.
